Column order
A B-tree stores entries sorted by its columns, left to right. Put the equality column first and the ordering column last, and the index hands rows back already sorted.

Queries filter on status and order by placed_at. Build one index so SELECT order_id FROM orders WHERE status = 'paid' ORDER BY placed_at LIMIT 20 has no Sort.
